Adding a Subject Guides feedback form

A pop-up feedback form for use in Subject Guides can be created in LibWizard by using the Tutorials function to create an embedded tutorial. Each Subject Guide requires a new form, edited with the title of your guide and your contact details for gathering responses. To see an example, take a look at the Black Studies Guide.

Create a feedback form using the template

How to create your feedback form using the template:

  • In LibWizard, go to Tutorials and Assessments
  • Open the Embedded Tutorial Overlays (Feedback) folder
  • Click on the Create New button
  • Click on the Tutorial Embedded in a Webpage radio button
  • From the Start With drop down menu, select TEMPLATE Subject Guides Feedback Form
  • Edit the name of your guide to [name of your guide] Subject Guide Feedback
  • Click Save - all feedback forms should be saved in the Embedded Tutorial Overlays (Feedback) folder

  • In the Slide Workpad, go to Edit Slide Content and change the title to [name of your guide] Subject Guide Feedback 
  • Under Website to Embed This Tutorial On select A Guide from LibGuides and select your guide from the drop-down list
  • Click Save

In the Settings, edit as follows:

  • Permissions: in Edit Permission add any additional editors by selecting Admins plus these accounts from the drop-down list and selecting the email addresses of additional editors.
  • Widget settings and code: copy the Widget Code here for use in the subject guide (button text, position, and colour are already set by the tempplate)
  • Email settings: in Email Notify change to your email address. Add any additional email addresses by separating with a space.

Adding the form to the Subject Guide

Follow these steps to add your feedback form to the subject guide. It will appear as a yellow button on the top left hand corner of the page.

  • ​​​​​​Add a standard box with the title Feedback form to the bottom of the right hand column of the Welcome page.
  • Tick the Floating Box check box so that it will display without borders (i.e. it will be invisible in public view).
  • Add Media / Widget
  • Give your widget a title e.g. Feedback form.
  • Copy and paste the widget code from the LibWizard Widget Settings into the box

Map this box to every page of the Subject Guide, to the bottom of the right hand column, so that the form remains constant on all pages. You will need to edit each additional mapped box by checking the Floating box option so that it remains invisible in public view. Please note: you will not be able to add the feedback form to a mapped page.
